doc(sql-migrate): add PGOPTIONS multi-tenant schema docs to help text

Document how to use PGOPTIONS="-c search_path=..." for multi-tenant
PostgreSQL migrations. Each schema gets its own _migrations table,
so tenants are migrated independently.
